Accessing someone else’s information (Proxy access)

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

Diabetic Eye Screening

You will usually be invited for diabetic eye screening every 1 or 2 years, invites are sent via letter.

Some people may need to be screened more often than that. Screening is different from your normal eye sight test for glasses.

 

Our Nursing Services

Our nursing team provides a wide range of health services. Appointments with a nurse or healthcare assistant can often be booked more quickly than with a doctor for routine procedures and health monitoring.

Chronic Disease Management

If you have a long-term (chronic) health condition, we will invite you for an Annual Review. The goal is to ensure your medication is working correctly and to help you stay as well as possible.

Cervical Screening Information

Cervical screening is one of the most important things you can do to protect yourself from the risk of cervical cancer. It is a free NHS health check that takes less than 10 minutes.

Child Immunisation Information

We provide all routine childhood vaccinations as part of the National NHS Programme. Keeping your child up to date with their vaccines is the best way to protect them from serious diseases like measles, meningitis, and polio.
