Sick notes

This document is to request a continuation of your sick note only.

Sick notes are only issued if you have been off work sick for 7 consecutive days AND you have had a routine consultation to discuss your illness with a clinican

If this is your first sick note request, please call the surgery to book a routine appointment to discuss your illness.

To request a continuation of a sick note:

• fill out a sick note request form
• phone or visit the surgery from 9am to 5pm

If you’re off work sick for 7 days or fewer, your employer should not ask for medical evidence that you’ve been ill.

Instead, they can ask you to confirm that you’ve been ill.

You can do this by filling in a form yourself when you return to work. We call this self-certification.
