Complaints Procedure
If you have any complaints about any aspect of our services please contact Mrs Toni Hartwell, Practice Manager in the first instance and we will do our utmost to resolve any problems amicably. If a complaint arises our aim is to explain any misunderstanding, to reconcile any disagreement or to make a commitment to change a faulty or ineffective practice policy.

Coventry and Warwickshire Integrated Care Record
Stockingford Medical Centre works with other health and social care organisations to share information that will form part of your Integrated Care Record. The Integrated Care Record allows health and care professionals involved in your care to view your records to help them understand your needs and make the best decisions with you, and for you. Information we hold about you will be available, to read only, to other Health and care professionals in Coventry and Warwickshire, Birmingham and Solihull, and Herefordshire and Worcestershire when they are involved in your health or social care.

Disabled Access
Access for Wheelchair Users
There is a ramp leading into the surgery for easy access for wheelchair users. There is also a bell situated near the front door if you require any help from the receptionists to access the building. There is a foyer located at the entrance that wheelchairs/motorised scooters can be left. The surgery does not accept any liability for damage or loss of any items left in the foyer. We also ask that access to the main doors are kept clear.
Disabled car parking
There are limited amount for disabled car parking spaces on our car park. Please use them sensibly and only park here if you need to.
Toilet for wheelchair users
There is a toilet available for our patients in the surgery. This has been adapted for our wheelchair users/disabled patients.
Please allow 2 weeks for any form to be completed.

Patient Confidentiality
We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible advice and care. This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care You have a right to know what information we hold about you.
If you would like to see your records please contact the surgery for advice. In certain cases there will be an administrative fee payable by the patient which will be advised prior to the release of the medical records.
Practice Policy
It is the surgery’s policy to maintain a no smoking environment for yours and our staffs comfort. Dogs are not permitted on the premises with the exception of guide dogs.
Summary Care Record
Information for new patients: about your Summary Care Record
Dear patient,
If you are registered with a GP practice in England, you will already have a Summary Care Record (SCR), unless you have previously chosen not to have one. It will contain key information about the medicines you are taking, allergies you suffer from and any adverse reactions to medicines you have had in the past.
Information about your healthcare may not be routinely shared across different healthcare organisations and systems. You may need to be treated by health and care professionals who do not know your medical history. Essential details about your healthcare can be difficult to remember, particularly when you are unwell or have complex care needs.
Having a Summary Care Record can help by providing healthcare staff treating you with vital information from your health record. This will help the staff involved in your care make better and safer decisions about how best to treat you.
You have a choice
You have the choice of what information you would like to share and with whom. Authorised healthcare staff can only view your SCR with your permission. The information shared will solely be used for the benefit of your care.
Your options are outlined below; please indicate your choice on the form overleaf.
- Express consent for medication, allergies and adverse reactions only. You wish to share information about medication, allergies for adverse reactions only.
- Express consent for medication, allergies, adverse reactions and additional information. You wish to share information about medication, allergies for adverse reactions and further medical information that includes:
your illnesses and health problems, operations and vaccinations you have had in the past, how you would like to be treated (such as where you would prefer to receive care), what support you might need and who should be contacted for more information about you. - Express dissent for Summary Care Record (opt out). Select this option, if you DO NOT want any information shared with other healthcare professionals involved in your care.
If you chose not to complete this consent form, a core Summary Care Record (SCR) will be created for you, which will contain only medications, allergies and adverse reactions.
Once you have completed the consent form, please return it to your GP practice.
You are free to change your decision at any time by informing your GP practice.
Translation
We can arrange interpretation and translation services for patients who do not speak English. Please let us know if you need this service when booking an appointment.
We also have access to the Coventry and Warwickshire sign language interpreting service for those patients who are hard of hearing and use sign language.
Violent and Aggressive Patients
We have a zero tolerance policy for those patients who are violent and aggressive against anybody in the surgery.
