HOW TO REGISTER

To register at our surgery all you have to do is:

  1. Come into surgery to collect the forms
  2. Bring it back with all the forms filled out
  3. Must bring 2 pieces of ID for yourself (proof of address, dated within 6 months and proof of ID)
  4. You will receive a letter if you have been accepted at our surgery; you MUST attend the New Patient Medical Appointment with the Nurse or Health Care Assistant to complete your registration.
